Survey Options

Illinois State University currently has three options for creating online surveys and forms for academic research and line of business use. This article can help you decide which option is best for your needs. Please use the links in each section to learn more about each option. 

Qualtrics

The Qualtrics Survey Platform is available to all Illinois State faculty, staff and students. To create or edit a survey in Qualtrics, a valid Illinois State ULID and password is required at login. 

Use the Qualtrics Survey Platform in endless ways from simple forms to complex research questionnaires.  With Qualtrics you can easily add from a large selection of question types like multiple choice, text entry, matrix, group, rank, drill down, file upload, and more. This tool also has very powerful analysis and reporting features which can help make sense of the data collected.

Microsoft 365 Forms

Microsoft Forms is available to all Illinois State faculty, staff and students through Office 365 Online. You can create surveys, quizzes, and polls, and easily see results as they come in. You can invite other to respond to it using any web browser, even on mobile devices. Surveys can be shared with participants beyond Illinois State. To create or edit a survey or quiz in Forms, a valid Illinois State ULID and password is required at login. 

You can create surveys, quizzes and polls through Microsoft 365 Online. You can see the results as they come in. Built-in analytics allow you to evaluate responses in real-time, allowing you to use forms to poll live audiences during a presentation or lecture. Data can be easily exported to Excel for additional analysis or grading.

Formstack

Formstack is primarily a data collection tool that website content managers can use for line of business purposes. It is available to Illinois State employees who have completed training on how to develop forms in Formstack. 

Formstack is a platform intended to assist administrative staff who want to build online forms that are embedded in Illinois State University websites.

Note

There are a limited number of Formstack accounts available. 

  1. Verify you have approval from your unit supervisor for a Formstack account
  2. Sign up for a Formstack short course session. 

Note

Formstack should not be used as a repository for data collection because the number of forms available is limited and additional forms add to the annual cost of this software. Considering using Microsoft Forms as an alternative and Free option for online data collection.
