Faculty and staff at Illinois State University may request access to the Formstack application.

Request Access to Formstack

Before a Formstack account is created, completion of the Formstack course is first required.

1. Navigate to Help.IllinoisState.edu/get-it-help.
2. In your ticket, write that you are interested in accessing the Formstack course. 
3. The Technology Support Center will enroll you into the Formstack course. You will receive an email stating that you have been enrolled.
4. Upon completion of the course, a ticket will be opened on your behalf. This ticket will request some information from you, such as the folder you need access to and official supervisor approval. 
5. Once your account has been approved by your supervisor, you will receive access to Formstack and the folder(s) you requested access to. 

Log into Formstack

Once you have been granted Formstack access, access the application by navigating to Forms.IllinoisState.edu. You will be logged in via Central Login. 

You have now successfully signed in and will be directed to your dashboard.
