Schedule an Appointment for the SHIELD COVID-19 Test

Applies To

Illinois State University students, faculty, and staff on campus who are not vaccinated are required to undergo COVID-19 saliva testing once per week. This article describes how to schedule an appointment to receive a SHIELD COVID-19 test.

Scheduling your Appointment

To schedule an appointment for COVID-19 testing, do the following:

  1. Go to

  2. Click the CentralLogin button and log in with your ULID and password.

  3. Click Schedule a SHIELD COVID-19 Test (Figure 1).

  4. Click Schedule an Appointment.


    You are required to acknowledge several agreements before you can make your appointment. Once you complete the required forms, you won’t be asked to do it again. If you see a message advising you need to do so (Figure 2), click the Forms link and follow the on-screen instructions and then start the appointment scheduling process again.

  5. Select SHIELD COVID-19 Test and click Continue.

  6. Review the instructions and click Continue.

  7. You are required to read and digitally sign the Consent for Treatment form. Review the agreement and digitally sign it by typing your name in the box. Then submit the form.

  8. Select a date and a location from the Location drop-down menu. You will go to this location for your SHIELD COVID-19 test (Figure 3).

  9. Click Search for appointments.

  10. A list of available appointment times is displayed. Select your desired appointment date and time and click Continue.

  11. On the final page, review the date and time you selected and make note of the location of the testing site. COVID-19 testing is performed at various locations throughout campus and will not take place at Student Health Services. When you are done, click Confirm.

You will see a message on the screen that says you have successfully booked your appointment, and will include a QR code. You will be asked to present this QR Code when you arrive at your appointment. You will also receive an email message with a copy of the QR Code associated with your appointment. The email message will also include the location of your appointment, along with the street address of the campus building where your appointment will be held.

Canceling your Appointment

You may cancel your appointment if necessary. To cancel your appointment, do the following:

  1. Go to

  2. Click the CentralLogin button and log in with your ULID and password.

  3. Click Schedule a SHIELD COVID-19 Test.

  4. Your currently scheduled appointments are displayed (Figure 4). Find the appointment you want to cancel and click Cancel Appointment.

  5. Select an option from the Reason drop-down menu and click OK.
